1. Submission Form

Cardinal Health offers a form for submitting papers to the Cardinal Health Hospital Efficiency Forum (“Forum”), by which we will collect and further process your personal data.

2. What personal information do we collect, what is the legal basis and for what purpose?

Registration and participation is on a purely voluntary basis and the personal data collected is necessary to process the submission of the papers.

The purpose of the processing of personal data for the Forum is to allow us to manage your paper submission and, where applicable your attendance at the Forum, and to publish on the Forum website the agenda and proceedings of the conference. Author names are published in the Forum programme. Personal data will also be used for transparency disclosure and employer authorization purposes in case of selected papers according to the paper submission terms and conditions.
